Drug Conversion from a Mixture of Fentanyl and Bupivacaine to Morphine in Intrathecal Drug Administration Pump: A Case Report

Abstract

Intrathecal drug injection emerged as an effective treatment for chronic pain patients. Chronic pancreatitis is accompanied by chronic abdominal pain. In a chronic pancreatitis patient with no effect by other treatment, intrathecal opioid might be a good treatment after the intrathecal pump was developed. A female patient visited to our pain center for refilling intrathecal pump. She has been injected with a mixture of fentanyl and bupivacaine. Because high concentration of fentanyl and bupivacaine were not permitted by Korea FDA, we refilled her pump to morphine alone. There was no complication and the patient was satisfied with morphine infusion. Intrathecal opioid administration is considered to be an effective treatment for chronic pain patients and it may be possible to control pain more effectively by mixing with high concentrations of fentanyl or bupivacaine. In addition, studies for the conversion of intrathecal bupivacaine to other drugs are needed to control the correct dose of pain control.
